RuffaloCODY  Wellness Incentive Program


Program Period: January 1, 2012- December 31, 2012


Incentive: $100 gift card


Wellness Program Requirements:

Employee must do the following in order to receive the RuffaloCODY Incentive:

1.  Participate in the Health Risk Assessment and Biometric Screening.     

  • Current employees to complete by Jan. 29, 2012
  • New employees to complete within 30 days of hire


2.  Participate in 3 Health Educational Programs. 

  • Multiple sessions will be offered on a quarterly basis throughout the year.  Sessions will be offered in person for on-site employees and via teleconference/WebEx for off-site employees


3.  Participate in 1 company-wide wellness challenge.   

  • 2 Intradepartmental wellness challenges will be offered throughout the year.


Exercise 12 times a month for at least 20-30 minutes. 

  • Self-reported activity.  Notify Mercy Wellness Coach of completion.


4.  Coaching Program (see below).


If an employee is in the “high risk” category, in addition to the criteria above, he/she must participate in the 12-week Mercy Corporate Wellness Coaching program. 

  • Sessions will be held every other week (6 sessions total).
  • Wellness Coach will be assigned to each high-risk participant. 
  • Sessions will be held face-to-face or telephonically on company time for employees.    
  • After the initial 6 sessions, the Wellness Coach will determine the frequency of future meetings based on client engagement, progress toward established goals and readiness to change. 

*Mercy will contact high-risk employees to schedule the first coaching session.
